The Community Health Center of Yavapai (CHCY) has been serving all of Yavapai County since 2001. Since its inception as the Prescott Free Clinic, Inc. in 1993, it has stayed true to its mission to contribute to the well-being of the people of Yavapai County by offering integrated primary health services including medical, reproductive health, dental, and mental health. With three full-time locations in Prescott, Prescott Valley, and Cottonwood, CHCY is the medical home for over 12,000 uninsured, underserved, and underinsured patients.
