About Homeless Health Care Project - Northeast Valley Health Corporation

The agency provides health services primarily for people who live in the San Fernando and Santa Clarita Valleys, including people who are homeless, mentally ill, undocumented, uninsured, and medically underserved. Services are provided at eleven locations; and include general medical care, including services as a community clinic and contract clinic for low-income patients under the L A County DHS PPP contract; CHDP examinations; dental services for children ages 17 and under; women's health; and family planning. Services are targeted, but not restricted, to the Latino/Hispanic community. Special services are targeted to teens, people who are homeless, and people who are HIV . Geographic restrictions apply for some services.

The Pacoima, San Fernando, LAC Canoga Park and LAC Valencia Health Center offer general medical care by appointment, and limited walk-in basis. Disease management programs are offered for hypertension, diabetes and asthma patients. General medical services include physical exams; minor special procedures; vision and hearing screenings; x-ray services; internal medicine; immunizations; CHDP exams; a well-baby clinic; pediatric care; prenatal/postpartum care; OB/GYN services; premarital blood tests; and confidential HIV testing and counseling.

Public/Private Partnership (PPP) provides outpatient health services for uninsured, low-income Los Angeles County residents of all ages, including General Relief recipients. It provides primary general medical care. Applicant's household income must be at or below 134% of federal poverty guidelines, or who are GR recipients.

CHDP provides regular, preventive health assessments and developmental assessments for children from birth to age 21 who are Medi-Cal eligible and for children from birth through age 18 whose family income is less than 200% of the federal poverty level. The check-ups are offered at set intervals and may include the required Head Start or State Preschool and pre-first grade exams.

The clinics are also a certified providers of Comprehensive Perinatal Services Program (CPSP), an elective Medi-Cal funded program which provides comprehensive pre-natal care including evaluation, direct medical services; nutrition and health education; psychological counseling and social services if needed.

Family planning services include pregnancy counseling and testing, birth control information and assistance, STD tests and treatment, pap smears, infertility information and assistance, sterilization information and services for teens.

Dental services for children are provided at the San Fernando Health Center and include examinations, cleaning, fillings, simple extractions and x-rays.

Immunizations for children and adults are offered at all NEVHC Health Centers.

Secondary services include dental services for adults at the San Fernando Health Center, and include examinations, cleaning, fillings, simple extractions and x-rays. New adult patients are only being accepted if they are homeless, pregnant, diabetic or chronically ill and they must be current patients of NEVHC Health Centers.

The agency also provides school based health services at L.A. Mission College, San Fernando High School and Maclay Middle School. Services are restricted to the students at the school, and its affiliated programs.

APPLICATION PROCEDURE

Call for information or to schedule an appointment; some services are available on a walk-in basis during clinic hours. People who call the Canoga Park or Valencia Health Centers speak to a triage nurse who may give callers instructions for self-care to follow at home. People who are homeless should request homeless health care services when applying for service. Call the San Fernando clinic to schedule a dental appointment. For school-based services call the designated number for information.



FEES/PAYMENT SOURCE

Fees are set on a sliding scale; many clients pay no fees. There are no fees for homeless health project services. Medi-Cal and private insurance are accepted. Eligible PPP patients pay no fees. Dental services fees are based on a sliding scale starting from $30 to $70 per visit, and depend on the type of service. People are asked to bring ID, proof of income, and their social security card. Staff can help people to apply for Medi-Cal, Healthy Families, Healthy Kids, and other low-cost managed health care programs.
