About Crescent Community Clinic

If you have a chronic health condition or a mental health diagnoses you may be eligible for free health care at Crescent Community Clinic.

Chronic health care and mental health services may be available if you are between the ages of 18 and 64, have no insurance, including Medicaid and Share of Cost, meet Federal Poverty Guidelines.

Intake and Eligibility

Come to the Clinic weekdays except Thursdays between 9:00 AM and 2:00 PM and bring a current photo ID, show proof of income or lack of income and be a Hernando County resident.

* A $5.00 registration fee is expected at time of application.

*No services will be provided for anyone abusing alcohol or illicit drugs.

Obamacare purchased from the Health Care Marketplaces covers Mental Health Care and Substance Use Disorder Treatment. Under the Health Care Law there are 10 categories of benefits. Mental Health Care and Substance Abuse Treatment are two of the 10.

If you have Affordable Care Act Coverage you will have mental health care coverage.
