Public Health services are provided at Public Health Centers located in Fulton County. Contact the Center nearest you for more information about services or call 404-612-1211 (voice). Not all services are offered at every Public Health Center.
Most Public Health Centers are open 8:30 AM to 5:00 PM, Monday through Friday. Click on a clinic below for evening hours, Saturday clinics, and services offered in the clinic.
Fees for services are charged based on a sliding fee scale based on income and family size. An individual with no health insurance can qualify for the sliding fee scale as long as you bring the following information:
1. Proof of Fulton County residency and
2. Proof of income and family size

What this means to you is that College Park Health Center - Fulton County Public Health Department provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
