This clinic provides a sliding fee discounts for self-pay patients who meet income eligibility requirements. Self pay patients may qualify for the discount program based on family size and proper documentation of all household income.
Monday, Wednesday, Thursday, Friday 8a.m.-5p.m. Tuesday 8a.m.-7p.m. Expanded Adult Medical Services are Monday-Saturday 8a.m.-8p.m.
