Hours: Mon - Fri: 8:00 am - 5:30 pm Services: Education on managing chronic illnesses such as diabetes and asthma, Group visits for diabetic patients, Mental health, nutrition and substance-abuse counseling, Behavioral health counseling -- on-site support for managing any health condition with a behavioral component, from anxiety and depression to diabetes, hypertension and more.
Also: Help with changes such as improving diet and exercise habits or reducing substance abuse, Help enrolling in benefit programs to obtain insurance, Medication assistance, On-site lab services, On-site dental care, Free breast and cervical cancer screenings for eligible women, Translation and child care available during appointments, and some transportation assistance may be available. Uninsured patients are offered a sliding fee scale based on their income, no one is ever turned away.
Families First Health Center operates under a sliding fee scale payment model. What this means to you is that Families First Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
