The Primary Health Network, a Federally Qualified Health Center, provides quality primary care services and access to specialty care commensurate with the needs of the people in the communities we serve. Services are offered regardless of age, race, creed, sex, national origin or ability to pay.
Primary Health Network accepts most major insurances including Medicare, Medicare Advantage Plans, Medicaid, Medicaid Managed Cares, Commercial Insurance, Private Insurance, W/C, and Auto Accident Insurance.
Sliding Scale Fee billing is based on income and is available to those who quality, regardless of insurance status.
Beaver Falls Primary Care & Behavioral Health Center operates under a sliding fee scale payment model. What this means to you is that Beaver Falls Primary Care & Behavioral Health Center provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.
