Mental Health Case Management
Mental Health Case Management Services are available for adults with problems relating to mental health. These services provide the following:
Evaluation: Individuals will initially be evaluated to determine eligibility and appropriateness for our services. The individual may receive a mental health evaluation from a certified psychiatrist.
Linkage: Once an individual has been enrolled in mental health case management services, they will be linked to services available in the community to meet their identified needs.
Coordination: We work together with family, physicians, the Criminal Justice System, therapists, the Department of Social Services, school staff and administration, and other service providers and agencies to work toward the common purpose of helping individuals reach their potential.
Advocacy: We provide a voice for individuals when their own voice may not be heard to empower them to strive toward personal goals.
Supportive Counseling: We provide support and encouragement, educate individuals on appropriate behavior, and conduct and teach valuable coping and communication skills. We encourage individuals to participate in their treatment and maintain treatment compliance actively.
Follow-up: We provide regular follow-up by meeting with the individual and coordinating with other agencies involved in their life at least monthly and throughout their treatment.

District 19 Community Services Board Mental Health Case Management is a certified Community Mental Health Center and therefore operates through federal and state funds made possible by the Community Mental Health Act. The purpose of these funds to make diagnosis and treatment available to low-income and indigent individuals. The services are either free or under a sliding-scale. These services are available to people of all ages who do not have health insurance.

District 19 Community Services Board Mental Health Case Management accepts Medicare, a federal health insurance program for people age 65 and older and people with disabilities.
Medicare covers a wide range of mental health services.
