You are responsible for your payment. We accept most major insurances, including most AHCCCS plans. We also provide a sliding fee scale for our private-pay patients, which is based on your verified income.

27th Avenue Health Center Terros Health is a certified Community Mental Health Center and therefore operates through federal and state funds made possible by the Community Mental Health Act. The purpose of these funds to make diagnosis and treatment available to low-income and indigent individuals. The services are either free or under a sliding-scale. These services are available to people of all ages who do not have health insurance.
The US Department of Health and Human Services offers funds in the form of Community Services Block Grants to all states. The purpose of these funds is to reduce poverty by helping low-income individuals and families find work, access education, obtain housing, and improve their health. 27th Avenue Health Center Terros Health is a recipient of this grant. You may qualify for assistance provided you meet low-income eligibility requirements but you must apply by contacting the following office:
