1. IDENTIFICATION
Adult - Bring one of the following for each related adult in the household Driver's license, I.D. card, passport/visa, immigration documents
Child - Bring one of the following for each child in the household Birth certificate, school record
2. PROOF OF INCOME
Bring at least one of the following:
Last month's paycheck stubs, welfare benefit documents (TANF, food stamps), Social Security documents, tax returns, Medicaid form
3. PROOF OF ADDRESS
Mail with your name, lease or house payment, voter registration, utility bill
