Horizon Health and Wellness operates under a sliding fee scale payment model. What this means to you is that Horizon Health and Wellness provides their services even to patients who cannot afford care (such as low-income or who people who need to self-pay). Patients will pay according to their income. Be prepared to provide information on your household income.

Horizon Health and Wellness receives federal grants destined for mental health services. This means that Horizon Health and Wellness either recieved money to obtain certification or training for its employees or it has recieved money to offer services such as suicide prevention, telehealth programs, or other behavioral initiatives.

Medicare Part B (Medical Insurance) helps cover mental health services that you would generally get outside of a hospital, including visits with a psychiatrist or other doctor, visits with a clinical psychologist or clinical social worker, and lab tests ordered by your doctor.

If you get your Medicare benefits through a Medicare Advantage Plan (like an HMO or PPO) or other Medicare health plan, check your plan’s membership materials or call the plan for details about how to get your mental health benefits.
All state Medicaid programs provide some mental health services. Horizon Health and Wellness accepts Medicaid, a joint federal and state program that helps with medical costs for some people with low incomes and limited resources. Medicaid programs vary from state to state.

If you have Affordable Care Act Coverage you will have mental health care coverage.
